Website enquiry forms collect the details you enter (name, contact details and your message).

- To process Medicare, CDBS, DVA and health fund claims
- To send appointment reminders and recalls
- To respond to enquiries submitted through this website
Storage & security
Clinical records are held in secure practice management systems. Website form submissions are stored securely and accessible only to authorised staff. We do not sell personal information, and we only disclose health information as permitted by law or with your consent.

You may request access to, or correction of, your records by contacting us at info@aligndentistry.com.au or (02) 9723 5757. We will respond within a reasonable time.
Complaints
If you have a privacy concern, contact the practice manager first. If unresolved, you may contact the Office of the Australian Information Commissioner (oaic.gov.au).
